إدارة النظافة الصحية اثناء الحيض

إهمال النظافة الصحية للحيض  في برامج المياه والصرف الصحي يمكن ان  يكون لها أيضا تأثير سلبي على الاستدامة.الفشل في توفير مرافق للتخلص من المواد الصحية  المستخدمة  يمكن أن يؤدى الى انسداد  المراحيض (HOUSE et al. 2012).

 

إدارة النظافة الصحية للحيض : الوسائل

المرافق المناسبة ومواد الحماية الصحية ليست سوى جزء من الحل. وبالإضافة إلى ذلك، فمن الضروري أن نتجاوز المسائل العملية لإدارة الحيض في المدارس وأماكن العمل، واستخدام الوسائل التعليمية , ويتيح  التعليم والمعلومات (بالاضافة الى   النظافة والتربية الجنسية) للنساء والفتيات من  معلومات واقعية حول أجسادهم وكيفية الاعتناء بها (كيرك وسومر 2006) (على سبيل المثال من خلال الحملات المدرسة أو جزء من المناهج الدراسية).وفى الوقت الحاضر، نادرا ما يتم تدريب المعلمين على  تعليم النظافة الصحية للحيض، وبالتالي نادرا ما يتم تدريس الحيض. قد يشعر المعلمون الذكور أن المعايير الثقافية تمنعهم من مناقشة هذه المواضيع مع الفتيات.ونتيجة لذلك، قد يتاخر  تدريس الإدارة الصحية لنظافة الحيض أو لا يدرس على الإطلاق (WSSCC 2013).

تمكين النساء والفتيات ايضا حتى  يكون صوتهم مسموعا و مراعاة الاحتياجات النظافة الصحية للحيض (انظر ايضا ورفع وعى الجمهور).لان نقص المعلومات الواقعية جراء انتشار الخرافات يعنى ان الفتيات والاحتياجات العملية المتصلة بإدارة النظافة الصحية  للحيض غالبا لا تقدر  او تعالج  على نحو مناسب (2006) كيرك & سومر (انظر ايضا اعداد المواد الاعلامية).

والنهج المجتمعية  يزيد  مع    تخطيط المجتمع خصوصا على اشراك الصبية والرجال، واعدة سبل تحسين ادارة النظافة الصحية للحيض  الحواجز المادية التي تواجه الفتيات والنساء بسبب عدم كفاية المرافق الصحية كثيرا ما تكون مرتبطة بحواجز اجتماعية مثل المحرمات ووصمة العار يلزم النظر فيها معا.

أداة أخرى لتحسين احترام كرامة المرأة هي أيام التوقف عن العمل أثناء وجود الحيض. في زامبيا، على سبيل المثال، يحق لكل امرأة عاملة بموجب القانون ليوم واحد إجازة من العمل كل شهر لضمان أن النساء تعمل في أفضل حالاتها. ويشار إلى هذا اليوم ب "يوم الأم".رغم عدم وجود تفاصيل، فان الاعتقاد الصامت ان منح امرأة يوم واحد إجازة في حالة وجود الحيض  يوم إجازة من العمل غير موثر للأداء الاقتصادى   كما يمكن للمرأة أن تجدد طاقتها  لتعوض يوم العطلة.

الايمان والخرافات والمحرمات

العديد من الثقافات لها المعتقدات، والخرافات والمحرمات الخاصة بالحيض.في معظم الأحيان تقريبا، هناك معايير اجتماعية أو قواعد و ممارسات غير مكتوبة  حول إدارة النظافة الصحية للحيض والتفاعل مع الحائض.بعضها مفيدة ولكن البعض الآخر لها آثار ضارة محتملة.على سبيل المثال، في بعض الثقافات، كما قيل النساء والفتيات ألا يستحممن أثناء الدورة الشهرية (أو أنها سوف تصبح عقيما)، المس بقرة(أو أنه سيصبح عقيما)،انظر في المرآة (أو أنه سوف يفقد لمعانه)، أوالمس النباتات (أو أنه سوف يموت) (هاوس وآخرون 2012).

غالبا ما تتافقم القواعد الثقافية والمحرمات الدينية على الحيض وتقترن تقليديا بالأرواح الشريرة والعار والاحراج المحيط بالتكاثر الجنسي على سبيل المثال: في تنزانيا إذا رأى احدهم الفوطة الخاصة بالحيض ربما يلعن صاحبة الفوطة (هاوس وآخرون 2012).

اكثر ما يلفت الانتباه أن كثيرا من النساء والفتيات يتعرضن  للرقابة والتقييد الذى  يؤثر على تصرفاتهم وانتقالاتهم بسبب هويتهم 'الجنابة'  أثناء الحيض، بما في ذلك الأساطير والمفاهيم الخاطئة والخرافات (الثقافية و / أو الدينية) المحرمات المتعلقة بدم الحيض والنظافة الصحية للحيض

يبين الشكل التالي أمثلة من هذه القيود في عدة بلدان آسيوية. وهناك قيودا مماثلة في البلدان الأخرى في جميع أنحاء العالم

ما وقد قلت ذلك، فمن المهم أن ندرك إمكانية الاختلافات البينية الثقافية في تفسير الحيض، وكيف أن 'المحرمات' في الواقع قد تخدم مصالح النساء، حتى لو كانت تظهر للوهلة الأولى أنها سلبية.على سبيل المثال، يمكن للمرأة أن تنظر  الى "النفي" فى أكواخ الحيض بايجابية لمنحهم فترة راحة من المهام اليومية (كيرك وسومر2006 ).

من اللافت للنظر أيضا أن التعليم من قبل الآباء المتعلق بالصحة الإنجابية والحياة الجنسية وجميع القضايا ذات الصلة يعتبر في كل مكان تقريبا بأنه منطقة محظورة ويبدو أنه في جزء كبير من آسيا وأفريقيا جنوب الصحراء الكبرى، مستوى الفتيات من المعرفة والفهم عن سن البلوغ والحيض والصحة الإنجابية منخفضة جدا (كيرك وسومر2006).

محرمات  الحيض تساعد في إلحاق الإهانة بالملايين من النساء والفتيات، ولكن الاسوء من ذلك :إن الافتقار الخطير من المرافق والمنتجات الصحية الملائمة يمكن أن يدفع الفتيات الحائضات الى الانقطاع عن الذهاب الى  المدرسة، مؤقتا، وأحيانا بشكل دائم (انظر أيضا مياه الصرف الصحي والجنس).

وصمة العار المحيطة بالحيض و النظافة الصحية للحائض هو انتهاك للعديد منحقوق الإنسان، الأهم من ذلك الحق في الكرامة الإنسانية، ولكن أيضا الحق في عدم التمييز والمساواة والسلامة الجسدية والصحة والخصوصية والحق في عدم التعرض للمعاملة غير الإنسانية والمهينة من سوء المعاملة والعنف (WSSCC 2013).

في الواقع، هناك علاقة بين النظافة الصحية للحيض و الغياب المدرسي للفتيات البالغات (الصف الرابع والخامس) من التعليم الابتدائي وما يلية  وتوكد البحوث  انه ببداية سن البلوغ يؤدى الى تغييرات كبيرة فى الالتحاق بالمدارس بين الفتيات.على الرغم من ان هدف تنمية الالفية  2 (التعليم الابتدائي للجميع ) تم انجازه في اشكال التعليم الابتدائي في العديد من البلدان النامية .  فان مشاركة الاناث ، ولا سيما في افريقيا واسيا، تتخلف كثيرا عن مشاركة الذكور في مراحل  التعليم الابتدائي والثانوي بالإضافة إلى حقيقة أن الفتيات يتزوجن في سن مبكرة في بعض الثقافات،العديد من الفتيات تبقى في المنزل عندما تبدأ في  الحيض، سواء بشكل دائم (الانقطاع المدرسي) او مؤقتة خلال ايام الحيض عندما تتخلف  الفتيات عن الركب  يمكن ان يؤدى ايضا ذلك الى الانقطاع عن الدراسة كما تخلق فترة الحيض الشهرية العقبات امام المعلمات. فقد يعلنون انفسهم  مرضى أو يعودون إلى المنزل بعد الدروس في أسرع وقت ممكن وليس لديهم ما يكفي من الوقت لإعطاء المزيد من الاهتمام للأطفال الذين هم بحاجة اليهم .وقد تودى الثقافة المدرسية غير الودية بين الجنسين والبنية التحتية وعدم وجود بدائل حماية الحيض الكافية و / أو مرافق صحية نظيفة وآمنة وخاصة للمعلمين والبنات  الى تقوض في الخصوصية، مما يودى إلى انتهاك حق أساسي من حقوق الإنسان من المعلمين والبنات .ونتيجة لذلك، فان  الفتيات والنساء الذين تخلينا عنهم لا يوجد لديهم تكافؤ في الفرص بسبب هذة  العقبة، الهدف 3 (تعزيز المساواة بين الجنسين وتمكين المرأة) لا يمكن أن يتحقق إبدا .(TEN 2007)

المخاطر الصحية من سوء إدارة النظافة الصحية للحيض

وهناك ايضا قضايا الصحة باستثناء القضايا الاجتماعية المذكورة اعلاه. ضعف الحماية وعدم كفاية مرافق للاغتسال قد تزيد الاستعداد للعدوى, ورائحة دم الحيض التى تعرض البنات لخطر الاهانة والوصم (انظر أيضا الصرف الصحي و المياه والصحة.في المجتمعات التي يمارس فيها ختان الإناث، يوجد مخاطر صحية متعددة. حيث الفتحة المهبلية غير كافية لتدفق الحيض، فبالتالى يحدث انسداد  وتراكم جلطات الدم خلف منطقة ختانهن . وهذا يمكن أن يكون سببا لفترة طويلة ومؤلمة، وزيادة رائحة، وعدم الراحة واحتمال العدوى الإضافية (كيرك وسومر 2006).

ويفترض ان خطر الاصابة (بما في ذلك الامراض التي تنتقل بالاتصال الجنسي) اعلى من المعتاد اثناء الحيض لان الدم يخرج من الجسم يخلق طريقا للبكتريا  للدخول الى الرحم. بعض الممارسات من المرجح أن تزيد من خطر العدوى (انظر الشكل أدناه). استخدام قطع القماش غير النظيفة على سبيل المثال، لا سيما إذا تم ادخالها  في المهبل، فتسمح بدخول البكتريا او تساعدها على النمو غير المرغوب فى والذى  يمكن أن يؤدي للإصابة.

على سبيل المثال، تظهر النتائج من بنغلاديش،80٪ من عمال المصانع هم من النساء 60%منهم قاموا باستخدام الخرق المحتوية على مواد كيميائية من أرضية المصنع لاستخدامها كأقمشة للنظافة الصحية للحيض ، ولذلك فان  73٪ من النساء العاملات لا تذهب الى العمل في المتوسط ستة أيام في الشهر,لا تملك المرأة مكان آمن سواء لشراء قطعة قماش أو فوط صحية أو لتغييرها / التخلص منها وعندما تدفع النساء النقود  لشراء الفوط الصحية في خلال أيام الحيض الستة فان هذا يمثل اضرار ا اقتصادية ضخماً عليها وأيضا لجدول مشترياتها التجارية(WSSCC 2013).

نادرا ما يذكر  في الدراسات التي أجريت في البلدان النامية عن المضايقات البسيطة، مثل آلام أسفل الظهر والانتفاخ، والتشنج، وتقلب المزاج، وأعراض أخرى تتعلق بالحيض التي تم توثيقها جيدا في الأبحاث الغربية.في حين أن الفتيات في البلدان المتقدمة عموما يستطيعون الحصول على مجموعة واسعة ذات طابع عام او محدد من المسكنات وغيرها من المنتجات الدوائية،الفتيات تعاني من أعراض مماثلة في أفريقيا و اسيا و  جنوب الصحراء الكبرى ولا يستطيعون الوصول إلى هذه "الكماليات"

مواد الحماية الصحية والتخلص منها

خيار الحماية الصحية إلى حد كبير قرار شخصي يستند الى  القبول الثقافي . وكثيرا ما تتأثر المرأة أو الفتاة بالبيئة والحصول على أموال وإمدادات المياه والخيارات بأسعار معقولة من الأهمية بمكان أن أي برنامج يهدف إلى دعم النساء أو الفتيات يجب ان ينطوي على مواد الحماية الصحية ويشركها في مناقشات وقرارات حول الخيارات المتاحة لدعم التخطيط (انظر أيضا اتخاذ القرار مع المجتمع المحلي و التخطيط مع المجتمع).

الفوط الصحية التي يمكن التخلص منها هي الطرق الأكثر استخداما من إدارة الحيض.في البيئات الفقيرة في الموارد التي غالبا ما تكون باهظ التكاليف، صعب نقلها ومن الصعب التخلص منها.العديد من النساء والفتيات المراهقات من الأسر الفقيرة لا تستطيع أن تشتري هذه الفوط الصحية (APHRC 2010). بل قد تلجا  بعض الفتيات للتجارة الجنس للحصول على مبالغ صغيرة من المال من أجل شراء مواد الحماية الصحية (كيرك وسومر 2006).لكن الفوط الصحية تقلل من العقبات المتعددة التي تمنع الفتيات من البقاء في المدرسة، الخوف من التلوث، والخوف من رائحة، وحتى لو كان هناك مرافق المياه والصرف الصحي في المدرسة، فالخوف من ظهور الدم بوضوح في المرحاض أو الحمام (WSSCC 2013).

الملابس أو الفوط القماشية   قد يكون الخيار الصحي المستدام، ولكن يجب أن تغسل صحيا ويتم تجفيفها في أشعة الشمس. ضوء الشمس هو معقم طبيعي وتجفيف الفوط القماشية على ضوء الشمس يعقمها لاستخدامها في المستقبل.كما انها تحتاج ليتم تخزينها في مكان جاف ونظيف لإعادة استخدامها. الفتيات الذين لا يعرفون ما هو الحيض يكون هناك  أمل ضئيل في إدارة النظافة الصحية للحيض بأمان أو صحيا، كما أظهر أحد المشاركين في ورشة العمل تجربتها الشخصية في النمو عندما كانت تعيش في  سيراليون: "أنا وأخواتي خبئنا  جميع الملابس الصحية التي لدينا تحت السرير لتجف بسبب الشعور بالعار " تجربتها  شائعة في جميع أنحاء العالم: يشارك العديد من المشاركين الحكايات من الدراسات الميدانية والمقابلات للفتيات والنساء الذين يحاولون تجفيف ملابسهم بعيدا عن الأنظار. في الممارسة العملية، وهذا يعني إخفائها في مكان رطب وغير صحى (اليونيسيف 2008؛ WSSCC 2013).

 كأس الطمث تقنية جديدة مناسبة للنساء والفتيات الفقيرات. وهو فنجان مصنوعة من المطاط سيليكون طبي يتم إدخالها في المهبل لجمع دم الحيض. أنه لا يحتاج إلى الإزالة التفريغ كثيرا مقارنة  بالفوط الصحية.أنه يقلل من المشاكل التي تواجهها الشابات التي تفتقر إلى الخصوصية ومرافق التغيير والتخلص من المنتجات الصحية في المدارس وفي سياقات أخرى قد توفر هذه التقنية بدائل مستدامة وعملية وفعالة من حيث التكلفة.من المستحسن أن عند استخدام كوب حيض واحدة يحتاج إلى الحفاظ على مستوى عال من النظافة خاصة أثناء إدخالها وإزالتها والنظافة الصحية .على الرغم من أن نقص المياه يمكن أن يشكل تحديات للاستخدام، كمية المياه المطلوبة عند استخدام كوب الحيض تكون في الحد الأدنى مقارنة بالأساليب الأخرى (أفرك 2010).توفير
أكواب الطمث يمكن أن تكون فرصة استثمارية  للقطاع الخاص، كما يتضح من التجارب من كينيا (روبي كأس 2013)

من منظور عملي جدا، الفتيات الذين يفتقرون إلى المواد الصحية الكافية قد يغيبن عن المدرسة كل شهر خلال فترة الحيض  (انظر أيضا الصرف الصحي و الماء والجنس).إذا الفتيات بالمدارس التي تفتقر إلى المراحيض الكافية وإمدادات المياه لتغيير المريح للمواد الصحية والاغتسال في خصوصية، وأنها قد تكون غير قادرة على البقاء بشكل مريح في المدرسة أثناء الحيض  (انظر أيضا الصرف الصحي والكرامة.عدم وجود مرافق الصرف الصحي النظيفة والخاصة التي تسمح للنظافة .الحيض قد لا يشجع الفتيات من الذهاب إلى المدرسة عندما تحيض. وتشير تقديرات اليونيسف (2005) أن حوالي 1 من كل 10 فتيات افريقية في سن المدرسة لا يذهبون إلى المدرسة أثناء الحيض أو ينقطعون عن الدراسة في سن البلوغ بسبب عدم وجود مرافق صحية نظيفة وخاصة في المدارس.

كما تؤثر المرافق الصحية السيئة في المدارس أيضا على خبرات المعلمات . ونظرا لعدم توفر البديل من المعلمين  بسبب نقص المعلمين في جميع أنحاء البلدان النامية ، وهذا يؤدي إلى انخفاض وقت التدريس للمعلمين بنسبة 10-20٪ (البنك الدولي 2005).

الفتيات التي تكون  قادرة أو عازمة على الذهاب إلى المدرسة طوال الحيض، ومع ذلك عدم كفاية المرافق والحماية الصحية قد يخلق عدم الراحة في الفصول الدراسية وعدم القدرة على المشاركة فى الانشطة الدراسية .على سبيل المثال، الفتيات الحائضات قد تتردد في الذهاب إلى أول الفصل للكتابة على السبورة، أو الوقوف للإجابة على أسئلة المعلمين، يرجع إلى الخوف من وجود 'حدث' وتلطيخ زيهم.

فمن الضرورى لإدارة النظافة الصحية للحيض عند الفتيات والنساء ،  الحصول على المياه والصرف الصحي (انظر أيضا الحصول على المياه والصرف الصحي.انهم يحتاجون الى مساحة خاص لتغيير مواد النظافة الصحية; والمياه النقية لغسل ايديهم والملابس المستخدمة; ومرافق للتخلص من المواد المستعملة او مكان لتجفيفيها إذا امكن  اعادة استخدامها.

Until now, poor menstrual hygiene in developing countries has been an insufficiently acknowledged problem. This paper draws attention to the relationship between menstrual hygiene and school drop–out rates for girls from the higher forms of primary (grade 4 & 5) and secondary education. Although its focus is predominantly sub-Saharan Africa, many of the issues raised are relevant to girls in most low-income countries, although there may be differences in popular practices and beliefs. Low Cost Handmade Sanitary Pads! From Design to Production. A Step Forward in Menstrual Hygiene Promotion in Pakistan

In order to manage the basic phenomena of menstruation, sanitary materials are used by women of all ages, almost all from 14 to 45 years of age. Though branded material is available in urban areas while difficult to access in rural, in those areas where such materials are available, they are expensive and difficult to afford and manage as well. It has been planned by IRSP to introduce Menstruation Hygiene Management (MHM) specific low cost technologies in Pakistan for not just providing ease in their practices but also for paving way for women empowerment through involving them in large-scale sanitary pad production.

Global Review of Sanitation System Trends and Interactions with Menstrual Management Practices

The problem with disposing of menstrual waste into pit latrines is that it causes the pits to fill up faster. The excreta in the pit decompose and decrease in volume, while the non-biodegradable components of menstrual waste accumulate and do not break down. Furthermore, once the sludge has been removed from the pit latrine, if it is to be used in agriculture, any waste that has not completely decomposed such as menstrual pads must be removed before the sludge can be composted or applied to farmland. The cost to remove, screen, and dispose of menstrual management products from pit latrine sludge is high and not accounted for.

We Can't Wait

This report is on the impact of poor sanitation on the safety, well-being, and educational prospects of women. Girls’ lack of access to a clean, safe toilet, especially during menstruation, perpetuates risk, shame, and fear. This has long-term impacts on women’s health, education, livelihoods, and safety but it also impacts the economy, as failing to provide for the sanitation needs of women ultimately risks excluding half of the potential workforce.

Is Menstrual Hygiene and Management an Issue for Adolescent School Girls? A Comparative Study of Four Schools in Different Settings of Nepal
